Eligibility

If you are new to Medicare, turning 65, retiring, moving, qualify for assistance, losing employer coverage, or have another qualifying situation, you may be eligible to enroll in or change your plan. UnitedHealthcare offers a variety of Medicare Advantage plans - one may be right for you. For all others, Medicare Open Enrollment is October 15 - December 7. What You Need to Enroll

You will need your Primary Care Physician's name and ID number     to enroll. To find this information, you can search for a provider now.

You will also need information found on your Medicare card.

Primary Care Physician's name and ID

Your primary care physician (PCP) plays an important role in your health care, and we want you to have the opportunity to select one you are comfortable with. Your PCP can:

  • Help you make good lifestyle choices to maintain and improve your health.
  • Serve as a direct point of contact in case of an emergency.
  • Manage the drugs prescribed to you and identify potential adverse interactions.
  • Coordinate the care of specialists who may diagnose and treat your conditions.
  • Help ensure that you are getting the care you need while helping to minimize your out-of-pocket costs.

Your Medicare claim number is located on your red, white and blue Medicare card.

You must have Medicare Part B to enroll in a Medicare Advantage plan. If you do not currently have Part B coverage, contact your local Social Security office or call the Social Security Administration toll-free at 1-800-772-1213 to enroll.

Medicare beneficiaries who meet certain criteria, such as moving outside of their current plan's service area, can change plans or return to Original Medicare outside of Open Enrollment.

This refers to the added benefits, or riders, that you selected. The monthly cost for these has been added to the premium amount.

Medicaid is a Medicare assistance program that helps you pay costs not covered by Medicare, such as premiums and deductibles. It may include additional benefits, such as prescription drugs, eye care and long-term care.

The Social Security/Railroad Retirement Board deduction may take two or more months to begin. If Social Security/the Railroad Retirement Board accepts your request for automatic deduction, premium withholding will not be retroactive and you will be responsible for paying for all premiums due from the enrollment effective date until the month in which premium withholding begins. If Social Security/the Railroad Retirement Board does not approve your request for automatic deduction, we will send you a payment coupon book for your monthly premiums.
