UnitedHealthcare® HouseCalls

A health and wellness program that comes to you

As part of your UnitedHealthcare® Medicare health plan membership, qualified members are offered our HouseCalls program. HouseCalls is designed to support and complement your regular doctor’s care with a visit to your home from one of our advanced practice clinicians. All of our clinicians are nurse practitioners, physician assistants or medical doctors.
A HouseCalls visit does not take the place of any of your doctor appointments – and is in no way meant to replace the care you receive from your regular Primary Care Provider.

Benefits of a HouseCalls visit

  • Convenience and Comfort – receive a yearly one-to-one health visit in your home or another agreed upon location. You are welcome to have a family member or other support person with you during your visit.

  • Personalization – appointments are driven by your needs. You can use your visit to prepare for a future appoint with your Primary Care Provider or help review the information from a previous appointment …the clinician is there to help you!

  • Dedicated time – the visit is 45-60 minutes and is for you and your needs.

  • Support – get access to and help coordinating additional resources to help you reach your health goals.

What to expect from a HouseCalls visit

  • One of our advanced practice clinicians will review your health history and medication(s), perform a physical exam, identify health risks and provide education on your health.

  • Time to discuss your health concerns or questions one-on-one with the clinician.

  • You will receive an Ask Your Primary Care Provider form, which you can bring to the next visit with your doctor.

How does it work?

You will receive a letter and a phone call from our HouseCalls team to schedule your HouseCalls visit. If you're not comfortable having the HouseCalls visit in your home, we recommend you ask a friend or family member to be there with you. Or, we can visit you at another location.

Once your HouseCalls visit is scheduled, you will receive an appointment reminder postcard before the appointment. All of the licensed health care professionals are contracted with UnitedHealthcare and have received specialized training regarding the health care needs of qualified members.
