Journey Forward (http://www.journeyforward.org/) is a collaboration among UCLA Cancer Survivorship Center, the National Coalition for Cancer Survivorship, Oncology Nursing Society, WellPoint and Genentech. These organizations began promoting survivorship care planning shortly after it was identified as a significant gap in care by the Institute of Medicine in 2005. To improve continuity and care coordination for cancer survivors, survivorship planning tools and educational resources were developed for the oncology and primary care provider communities and survivors.
The Survivorship Care Plan Builder is a free tool that was designed to assist oncology professionals in quickly creating custom Survivorship Care Plans in accordance with survivorship guidelines of the American Society of Clinical Oncology (ASCO®). Each Care Plan can be printed, emailed or stored electronically.

My Care Plan is a tool for survivors to initiate their own survivorship care plan in collaboration with their oncology team. Survivors complete the listing of care team members and self-assessment, and talk with providers to complete and check the overall care plan. An iPhone app for My Care Plan is available.
The Survivorship Library database containing many articles (including peer-reviewed) on survivorship care and links to credible resources. oncology professionals and survivors can go to the library.
